Free 1-hour parking is available on Green Street (2 hours free if east of Wilson). Paid parking is available behind our building (75 cents every 15 minutes, max $8.00).
Location
We are located on the 2nd floor above Pasadena Optometry, near the corner of Green and Catalina. From the parking lot you can take
the stairs to the left of Pasadena Optometry or the elevator next to Michael's Pharmacy.
Insurance eligibility. As a courtesy, we will bill the insurance you provide to us. Please verify with your insurance carrier prior to your
appointment that our office is in network with your plan. You are responsible for understanding your benefits; including what services will or
will not be covered and any special facilities you need to go to for your yearly physical, labs, immunizations and any tests our providers may
order.
Copays are due at the time of visit. Cash, check and most credit cards are accepted.
Cancellation policy Late cancels, no shows and late arrivals are subject to a $25 fee (office visit) or $40 fee (yearly physical) which may be
waived the first time only.
